Based on what you wrote above, I understand that you claim that the index.php file which is normally present to the site's root is never written to disk. How did you determine that this is the problem?
This file, along with all the other files of your site are backed up (placed into the backup archive) at backup time. If the file cannot be read at backup time it produces a warning. Do you see any such warning?
When you run Kickstart it extracts all of the files present in the archive to your server. If it could not write the index.php file it would have told you and refused to proceed with the extraction. Did something like that happen?
Or do you mean something completely different? If so, what?
