I've installed the actual version of Akeeba Backup (3.2) on a Joomla 1.5.17 (Windows NT), PHP 5.2.14.

Configuring the backup, there are the some fields missing!
(See attachment).

What's wrong?

Your browser has cached an old version of the Javascript file which renders the view. Please try clearing your browser cache and refresh the page. The fields should be there.
