Automatic extension updates are one thing.
Receiving scheduled emails with site reports is a different thing.

I am experimenting with Panopticon and looking at what the best settings for me. Is it set up with Akeeba Panopticon, if there is a new version of an extension, that a backup of the website is first made, before the new version of the extension is installed. As for the installation, this is quite possible, provided a backup is made first.
For extensions, no, taking a backup before is not implemented for practical reasons. Sure, for small sites on oversized hosting packages it would be no big problem. Most sites, however, are not like that. Backups need to be taken during periods of low usage so as not to adversely impact the visitors' experience.
For CMS updates, yes, taking a backup before installing an update is implemented because installing CMS updates can be deferred to a specific time of the day when you know traffic is sparse.
