Not sure if this is related to Akeeba Admin Tools or some other configuration issue with J4 itself.

I've reconfigured my site so that newly registered users have to be approved by the administrator to help mitigate spam. I'm the only administrator on the site, so super admin. Let's say I'm already logged into the front end of my site using my super admin credentials, so I should be set. I'll get an email from a new registration, and I'll click on the link at the bottom of the email to approve the new registration. I'm then taken back to my site and I have a modal saying "You don't have permission to access this. Please contact a website administrator if this is incorrect.". Since I'm already logged in as a super admin, is this a configuration issue with Admin Tools or is this something else.

This is Joomla! itself, not Admin Tools. This is the message you get when the anti-CSRF token is incorrect and indicates a session issue, even something as simple as not being logged in yet.

While not obvious, Joomla! expects you to already be logged into your site's backend before using the links in the emails concerning user activation.

Do keep in mind that the frontend and the backend of your site are two separate applications. By default, they act completely independently. You can be logged into the frontend and not be logged into the backend or vice versa. Joomla's unified sessions feature does some trickery to try to maintain the same login status in both application, while also maintaining two different sessions for the two different applications, and for good reason (anything else would break all components with both a front- and a backend presence, i.e. 99% of components out there, including all core Joomla! components).
