We have excluded the entire directory /dev when we back up our main website.
The question is do we also need to separately exclude the DB for the /dev site?
Everybody will be able to see its contents. Do not include usernames, passwords or any other sensitive information.

Yes, you have to. Files and database exclusions are completely separate.
