Everybody will be able to see its contents. Do not include usernames, passwords or any other sensitive information.

You don't have to. Akeeba Backup automatically puts a .htaccess file in the backup output directory to prevent direct web access to it.
Ideally, you should use a backup output directory outside your web root.

No, by default you should NOT see it. All dot-files (files whose name start with a dot) are hidden files on every Operating System except Windows. The default configuration of FTP clients is to not display hidden files.

No. If you want to keep the old backup archives on your server (NOT recommended) you need to leave the old output directory where it is. You can create a new directory and change to it in the Configuration page of Akeeba Backup. Old backups do store the absolute filesystem path of where the backup archive were located in so that Akeeba Backup can still find them even if you change the Output Directory in the Configuration page.

I just put a screenshot from my trusted Cyberduck that always shown dot files like htacces
