Everybody will be able to see its contents. Do not include usernames, passwords or any other sensitive information.

From the documentation:
"Extensions are case sensitive; this means that pdf, PDF and Pdf are three different extensions."
However it is impossible to enter more than 1 of the three versions. I guess when entering the case sensitivity is not tested. Is this a bug or am I doing something wrong?
You found a bug in Joomla itself :)
We are using Joomla's “fancy field” (that's literally what they call it) to allow you to enter file types. Unfortunately, it is treating your input as case–insensitive. We'll see if there is a way to beat it into submission or change the generated .htaccess code to be case insensitive.
