I've turned on the option to limit administrator access to certain IP addresses. This works well when using /administrator. However, when someone goes to /index.php?option=com_users&view=login or uses our front end login form, it does not limit their login by IP. How can I do this? I don't want a login form accessible outside certain IP ranges.

The whitelist feature was intended to restrict access to the back end of your site. It really can't be used to limit access to the front end. If your aim is to restrict certain geographic regions, then the GeoIP Restrictions might work for you.

If you want to limit to numeric IP ranges, then you can do that in the .htaccess file. You should be able to Google for examples of how to set it up. If you are using .htaccess Maker, you would need to put the command in the Custom commands fields so you don't lose them when you generate a new .htaccess file.
