Description of my issue:
Awesome Documentation. I followed the above link to the letter. Used Google Chrome with my Mac (thanks for including shortcuts on Mac Keyboard). In the documentation you mentioned 404 errors. I just wanted to say that in my case it was a 304 error.

I am using cloudaccess with cloudbase 2.0 template which is based on the gantry framework. Just like you said, the error was in the php file for the gantry part of the CSS. Only in my case it wasn't a 404 error. I had a 304. Just thought I would post for anyone in the same boat. I thought I would have to do a system restore. Thanks be to God I don't! I'm a total noob. The documentation you wrote is the bomb!

HTTP 304 is a "Not modified" message and does not indicate an error. It simply means that your browser is fetching the file from its cache. You must clear your browser's cache or try a forced reload (CMD-SHIFT-R in Google Chrome running on Mac OS X) when doing this kind of .htaccess debugging. In this case you simply got lucky :)
