Description of my issue:
When do i enable optimization in sh404 pages are 404, do i have to create .htaccess after enabling sh404 optimization?

This is a support request for sh404SEF, not Admin Tools. Anyway, I happen to know the answer even though I am not the developer of sh404SEF :) In order for sh404SEF (or any other SEF solution) to work at all you need to have a .htaccess File. You can either rename Joomla!'s htaccess.txt file to .htaccess or use Admin Tools' .htaccess Maker to produce a proper .htaccess (recommended).

But, i have another question in regards to admin tools. in SEO and Link Tools
Under Combine JavaScript and CSS
I tried yes, both methods plugin & direct.
I get blank page after saving.
Do i have to create .htaccess after saving

Try disabling the CSS option. Optimising CSS files is a CPU and memory intensive process. Many servers will timeout or run out of memory, throwing a blank page, while they are optimising the CSS files. There is no workaround to it, sorry :(

what about combining javascript

This is usually much less resource intensive and works on all but a very few hosts. It won't work on sites where the memory consumption is very close to the PHP memory limit. That's why I recommend turning off the CSS combination first (very resource intensive). If that doesn't work, you'll also have to turn off Javascript combination.
